About Project Concern Zambia (PCZ)

PCZ is a local Zambian organization affiliated with Project Concern International (PCI), a Global Communities Partner, an INGO operating in Zambia since 1996. PCZ strengthens the capacity of community-based organizations and Non-Governmental Organizations and collaborates with the Government Republic of Zambia (GRZ) to address the health and socioeconomic needs of vulnerable children, adolescents and their primary caregivers and make them resilient to overcome hunger and hardship.

Project Summary

Funded by the United States Agency for International Development (USAID), with support from the President’s Emergency Plan for AIDS Relief (PEPFAR), the Empowered Children and Adolescent Program II (ECAP II) is implemented in 16 districts in Central and Southern Provinces. The aim of the program is to improve the resilience and health outcomes among Vulnerable Children and Adolescents and their primary caregivers.

PCZ seeks to recruit four (4) Regional Case Management Coordinators to be based in Chikankata, Choma, Kazungula and Mkushi Districts

Job Summary

The Regional Case Management Coordinator (RCMC) will report to the Provincial Coordinator and will work with dotted line reporting to Health and Social Linkage Technical Officers to coordinate and supervise district level HIV/health and social services bi-directional referrals. S/he will supervise and provide mentorship to facility-based Case Managers and community-based Case Workers to ensure VCAs are:

(i) Identified in line with set criteria (ii) Enrolled into the USAID ECAP II program (iii) Linked to appropriate facility-and community-based HIV preventive and care services and other social services, and (v) Retained on the program until graduation. The RCMC will closely collaborate with Government community and district level health and social service structures such as primary health facilities and community welfare assistant committees to ensure standardization of referral protocols and seamless bi-directional referrals.

Specific Responsibilities

  • Participate in the development and routine refinement of program standard operating procedures (SOPs), case management protocols, bi-directional referral tools, and vulnerability assessment tools
  • Together with the Community Services Linkages Manager, Provincial Coordinators and Health and Social Linkage technical officers, develop a district-wide supervisory and mentorship system for Case Managers and Case Workers
  • Coordinate with service delivery partners to provide routinized supportive supervision and mentorship to Case Managers and Case Workers in line with the supervisory and mentorship guide and protocols
  • Continuously track challenges and best practices in the bi-directional referral system and timely implement corrective action in partnership with service delivery partners and stakeholders.
  • Share documented challenges, corrective action implemented, and best practices with the program management unit
  • Inform the continuous iteration of the program workplan to ensure the program strategy is continuously adapted to address emerging needs and challenges in the bi-directional referral system
  • Participate in district continuous quality improvement (CQI) activities and support the documentation of findings and development of corrective action plans
  • Support district data associates and provincial Strategic Information Officers to collect and document accurate linkage and case management data and support service delivery partners and stakeholders to analyze district-level data, and timely use data for decision making
  • Participate in district-level multi-sectoral VCA service delivery coordination meetings as advised by the Provincial Coordinator
  • Prepare and submit to the Provincial Coordinator weekly quantitative reports and monthly quantitative and qualitative reports in coordination with service delivery partners

Qualifications and experience

  • Degree or Diploma in Social Work, Nursing, Clinical Medicine, Counseling or any Other Social or Health-related field
  • At least one-year experience in HIV/AIDS community-based case management, OVC para social work, Community PMTCT and cohort case management of Mother Infant Pairs, or DREAMS case management
  • Experience working with Community/ Facility Case workers or Community Based volunteers
  • Experience working with literate and non-literate audiences
  • Experience working in health facilities to support community-health facility continuum of care services or experience working in DREAMS centers
  • Experience in supervising field teams
  • DREAMS Program graduate will be an added advantage
  • Willing to travel extensively within the district
  • Good communication and interpersonal skills
  • Computer skills in basic applications such as MS work and MS excel
